PopHealthCare’s National Value-Based Medical Group Emcara Health Joins Moving Health Home Coalition

Emcara Health, a national value-based medical group launched by PopHealthCare, announced today it has joined Moving Health Home, a coalition of stakeholders working to move more healthcare services into the home setting. Emcara Health is a recent expansion of PopHealthCare, a leader in home-based care and prospective/retrospective risk adjustment services.

Formed in March 2021, Moving Health Home members are working to change federal and state policies to enable the home to be a clinical site of care. The Washington D.C.-based advocacy group includes Amazon Care, hospital groups and in-home care providers such as Emcara Health who want to increase the availability to high-quality home-based healthcare services.

The innovative new care model from Emcara Health dramatically expands the reach of in-home care beyond the strictly homebound. The model can serve up to 20 percent of a Managed Care Organizations population, depending on the Line of Business, with the largest concentration found in Medicare Advantage plans. Those Medicare Advantage members eligible for Emcara services typically have more than three chronic conditions and account for up to 50 percent of all claims.

As a national, value-based medical group, Emcara serves all lines of business, including Medicare Advantage, ACA, Managed Medicaid, and Managed Medicare . Emcara also offers health plans the flexibility of choosing between a variety of care solutions, including, advanced primary care, advanced supportive care (currently known as PopHealthCare’s CareSight offering), and short-term care solutions (in-home assessments, social determinants of health (SDOH) assessments, transition of care, ER diversion and in-home palliative care).
